Abstract

Catheter ablation of an accessory pathway at the right free wall is sometimes challenging because of catheter stability or anatomical complexity. A 20-year-old male patient was referred to our hospital. Electrocardiography (ECG) showed Wolff-Parkinson-White syndrome with atrial fibrillation. Electrophysiologic study showed the accessory pathway was located at the right free wall. However, catheter ablation was failed in spite of many times of radiofrequency energy delivery. We decided to perform surgical ablation. After surgical ablation was performed successfully, ECG showed no delta wave even during atrial pacing and ventriculoatrial dissociation during ventricular pacing.
